BEFORE YOUR VISIT
Please fill out your health information online through our Patient Portal. Your information is strictly confidential and used to determine the best treatment for you.
If you have completed the requested forms online, please arrive 20 minutes before your appointment time. If you had difficulty filling out these forms online, our staff are happy to assist you — but please arrive 45 minutes before your appointment time to make sure everything is completed before a doctor sees you.
Make sure to bring your health insurance card. You are responsible for your co-payment at the time of your visit. Payment may be made with a credit card.
Check that your primary care physician has processed a referral authorization and that the authorization is cleared through our office prior to your appointment. If you are self-referred, please contact our office so we can assist you in determining if you need a referral authorization.
If you are unable to make it to your scheduled appointment, please contact us at least 48 hours before your appointment to reschedule.
DURING YOUR VISIT
In order to provide the best possible care, you will be asked many personal questions and the expert physician you see will listen carefully to best determine the root cause of your symptoms. Please be advised that your initial visit may take more than an hour.
A friend or family member is welcome to accompany you.
