Make sure to bring your health insurance card. You are responsible for your co-payment at the time of your visit. Payment may be made with a credit card. Check that your primary care physician has processed a referral authorization and that the authorization is cleared through our office prior to your appointment. If you are self-referred, please contact our office so we can assist you in determining if you need a referral authorization. If you are unable to make it to your scheduled appointment, please contact us at least 48 hours before your appointment to reschedule.
